How to Recognize and Respond to Early Heat Illness on the Trail
A practical guide to spotting early heat illness on desert trails, taking immediate action, and planning hikes to reduce dangerous heat exposure.
Heat illness often starts with symptoms that are easy to dismiss: an unusual headache, slower thinking, irritability, heavy sweating, or a sudden loss of energy. On a desert trail, waiting to see whether someone “pushes through” can turn a manageable problem into a medical emergency.
I treat any change in how a hiking partner is moving, speaking, or making decisions as useful information. Early action is simpler than rescue, especially when you're far from a road or reliable mobile service.
Recognize the early warning signs
Heat illness exists on a spectrum. A person can move from heat stress to heat exhaustion and then to life-threatening heat stroke, particularly during exertion, high temperatures, or limited access to water and shade.
Early signs can include:
- Unusual tiredness or weakness
- Headache
- Dizziness or light-headedness
- Nausea or vomiting
- Muscle cramps
- Heavy sweating, or sweating that seems excessive for the effort
- Thirst
- Irritability, confusion, or poor judgement
- A faster-than-usual pulse or breathing rate
- Clumsiness and a noticeable drop in pace
A person may not recognize these changes in themselves. Heat affects judgement, and people often become focused on reaching the next landmark or campsite. Ask direct questions, and pay attention to the answers. Someone who suddenly can't remember the route, gives muddled responses, or makes unsafe decisions needs more than a quick water break.
Sweating isn't a reliable way to rule out serious heat illness. A very hot person may still be sweating, while a person with advanced heat stroke may have hot skin without obvious sweating. The more important warning signs are changes in mental status, coordination, and behaviour.
Know when heat illness is becoming an emergency
Heat stroke is a medical emergency. Suspect it when someone has a markedly altered mental state—such as confusion, unusual behaviour, slurred speech, a seizure, collapse, or unconsciousness—especially after heat exposure and exertion. A very high body temperature may be present, but you shouldn't delay action while trying to measure it.
Other serious signs include repeated vomiting, inability to drink, worsening weakness, or symptoms that don't improve promptly after cooling and rest. Heat stroke can occur even when the person is still sweating.
When symptoms stop looking ordinary: If your partner is confused, collapses, has a seizure, can't drink, or is getting worse, treat it as an emergency. Call 911 in the United States or use your satellite communicator's emergency function, then begin cooling while arranging evacuation.
Heat illness can also resemble other problems, including dehydration, altitude illness, infection, low blood sugar, and hyponatremia. Hyponatremia—low blood sodium—can cause headache, nausea, confusion, and seizures, particularly after drinking excessive amounts of plain water. Don't force a sick person to keep drinking large volumes. Prioritize cooling, emergency advice, and evacuation when symptoms are severe or unclear.
Take immediate action at the first warning signs
At the first sign that heat is affecting someone, stop the activity. Don't wait for the planned rest stop, viewpoint, or campsite.
1. Move out of the heat
Get the person into deep shade or an indoor, cooled space if one is nearby. In the desert, a small patch of shade can make a meaningful difference. Use a shelter, umbrella, tarp, emergency blanket positioned to reflect heat, or any other practical shade source.
If you're on an exposed route, the safest shaded place may be behind a rock or below a ledge—but check for rockfall, flash-flood hazards, and unstable ground before settling there. Don't move farther from help simply to find a more attractive campsite.
2. Remove unnecessary layers and cool the body
Loosen or remove extra clothing and packs. Wet the skin with cool water and improve airflow with a fan, hat, map, or folded clothing. Place cool, wet cloths at the neck, armpits, and groin if available. Pouring water over clothing can help, but don't use up your entire supply without considering the distance to assistance.
For suspected heat stroke, cool first and keep cooling while help is on the way. Immersion in a cool stream or bath can be effective where it is safe and permitted, but don't risk drowning, strong current, contaminated water, or a dangerous scramble to reach it.
3. Offer fluids only if the person is alert
If the person is awake, able to swallow, and not vomiting, offer small, frequent sips. Water may be appropriate, and a suitable electrolyte drink or oral rehydration solution can help replace fluids and salts lost through sweat. Avoid forcing fluids, especially if the person is confused, drowsy, vomiting repeatedly, or having trouble swallowing; they could aspirate.
Don't rely on salt tablets or highly concentrated drink mixes as a universal solution. They can cause problems when used incorrectly and won't fix dangerous overheating. Cooling and getting medical help matter more than finding the perfect drink.
4. Reassess instead of rushing back onto the trail
Stay with the person and monitor their breathing, alertness, coordination, and ability to drink. Improvement should be clear, not merely a promise that they feel “a bit better.” If symptoms return when they stand or walk, the trip is over for that person.
Once someone has had significant heat symptoms, don't send them ahead alone and don't assume a short remaining distance is safe. Arrange a conservative exit, and ask another member of the group to accompany them if it can be done without putting others at risk.
Respond to suspected heat stroke
If the person is confused, unconscious, seizing, or rapidly deteriorating:
- Call 911 or activate your satellite emergency device.
- State that you suspect heat stroke and provide your location, route, group size, and the person's condition.
- Start aggressive, safe cooling immediately.
- Remove excess clothing and equipment.
- Keep monitoring breathing and responsiveness.
- If they're unconscious but breathing, place them in a position that helps keep the airway clear while continuing to cool them.
- If they aren't breathing normally, follow the dispatcher's instructions and begin CPR if you're trained to do so.
Don't give food or drink to an unconscious or confused person. Don't leave them alone to seek help unless there's no other way to raise an alarm, and only after considering the safety of the rest of the group.
Emergency response times in the Desert Southwest can be long. A satellite message, trailhead contact, park ranger, or nearby group may be the fastest route to assistance, but none replaces calling emergency services when a life-threatening condition is suspected.
Reduce heat exposure when planning the trip
The best response to heat illness is often changing the itinerary before the hike begins.
Check the forecast and route exposure
Look beyond the high temperature. Review overnight lows, hourly temperatures, humidity, wind, sun exposure, elevation gain, water availability, and the amount of exposed trail. A route that feels manageable in the morning can become dangerous after shade disappears.
Plan shorter distances and more conservative turnarounds during heat events. A map may show a water source, but it doesn't guarantee that the water is flowing, safe to drink, or accessible without a detour. Carry enough water for the route, expected heat, delays, and an emergency margin, and bring a treatment method for any water you intend to collect.
Start early and schedule the hardest work wisely
In hot desert conditions, begin before the hottest part of the day and use cooler hours for climbs and exposed sections. Rest in shade before you're exhausted, rather than waiting until everyone is struggling. An early start isn't a licence to ignore the afternoon forecast; turn around when conditions or symptoms change.
Acclimatize gradually
Heat tolerance takes time to develop. The first several hot days of a trip deserve shorter outings, lower pack weights, and more frequent breaks. Fitness doesn't eliminate heat risk, and previous success in hot weather doesn't guarantee that today's conditions are safe.
Use clothing and equipment that support cooling
A broad-brimmed hat, sunglasses, breathable clothing, sunscreen, and a light long-sleeved layer can reduce direct sun exposure. Carry a way to create shade, and keep your water accessible rather than buried at the bottom of your pack. A reliable navigation device and communication method are safety equipment, not luxuries, on remote routes.
Use a buddy system
Agree on symptoms that will end the hike before anyone sets out. Check in at regular stops: how is everyone feeling, how much water remains, and how far is the next reliable shade or exit? Speak up early if you're developing a headache, nausea, unusual fatigue, or confusion. A short, uncomfortable conversation is preferable to a delayed evacuation.
Before you leave the trailhead: Confirm current weather alerts, route closures, water-source conditions, park or land-manager requirements, and your emergency communication coverage. Regulations and water availability vary across the Southwest, so check the relevant agency or park service for today's route.
Practical next steps
For your next hot-weather trip, mark shade, water, and exit points on the route before you start. Carry enough water and a treatment option, pack a communication device, and tell someone at home your itinerary and return time.
Most importantly, make an early stop feel routine rather than embarrassing. If a person is showing signs of heat illness, cool them, reassess honestly, and be prepared to end the trip. On a remote trail, turning around at the first warning sign is a sensible decision—not a failure.
